Determinants of clinical practice guidelines’ utilization for the management of musculoskeletal disorders: a scoping review

Abstract Context Many clinical practice guidelines have been developed for the management of musculoskeletal disorders (MSDs). However, there is a gap between evidence-based knowledge and practice, reasons are poorly understood. Understanding why healthcare providers use essential to improve their implementation, dissemination, adherence. Aim To identify determinants guidelines’ utilization by health care involved in assessment MSDs. Method A scoping review literature was conducted. Three databases were searched from inception March 2021. Article identification, study design, methodological quality, type providers, MSDs, barriers facilitators associated with extracted selected articles. RESULTS: 8671 citations retrieved, 43 articles selected. 51% studies Europe, most quantitative (64%) following cross-sectional design (88%). Almost 80% dealt low back pain guidelines, studied general practitioners or physiotherapists. Five main guideline expressed providers: 1) disagreement recommendations patient expectations; 2) not specific individual patients; 3) unfamiliarity “non-specific” term, bio psychosocial model MSDs; 4) time consuming; 5) heterogeneity methods. Four cited: clinician’s interest practice; perception clinicians that will triage, diagnosis management; efficiency; standardized language. Conclusion Identifying modifiable first step developing implementation strategies practice.
